THE DR LIZ GERIATRICS TEAM

Dr Liz FOUNDER

Elizabeth Landsverk MD

Elizabeth (Dr Liz) has over twenty years of experience in providing medical care to the elders. She is board-certified in Internal Medicine, Geriatric Medicine and Palliative Care Medicine. Dr Landsverk founded ElderConsult Geriatric Medicine, a house calls practice, to address the challenging medical and behavioral issues often facing older patients and their families.

Dr Landsverk was an assistant professor of Medicine at University of California, San Francisco, an adjunct professor of Medicine at Stanford University, as well as consultant to the San Francisco Elder Abuse Forensics Center and Hospice Medical Director. She is currently the Medical Director for Silverado and Kensington Dementia Care Communities, as well as on the Scientific Panel for the Alzheimer’s Association. Dr Landsverk graduated from Stanford University, and trained at Cambridge Hospital, Harvard University, and Mt Sinai Medical School.

As a House Calls Geriatrician, she collaborates with local physicians to address the needs of complicated vulnerable elders to alleviate pain, agitation and discomfort through the utilization of geriatric and palliative care techniques.

Michelle Dhanak MD

Michelle has been a board certified Geriatrician for almost 25 years. She trained at the University of Massachusetts Medical School and the Highland General Hospital in Oakland. Prior to joining ElderConsult, Michelle developed a house call program for New Zealand. Dr Dhanak has extensive experience caring for people with dementia. She believes in a holistic approach and the importance of partnering with patients and their families in promoting dignity, respect, autonomy and self-efficacy for older individuals. She has a particular interest in utilizing physical activity to maintain physical and cognitive health. Dr. Dhanak is a fellow in the American College of Physicians and the Royal Australian College of Physicians.

DEMENTIA CONSULTANT AND EDUCATOR

Tami Anastasia MA

Tami has over 25 years of counseling, educating, and providing emotional support to families caring for loved ones with dementia. She has a Masters Degree in Counseling from Santa Clara University and is a dementia educator, an Alzheimer’s and dementia consultant, an Alzheimer’s caregiver support group facilitator and provides monthly educational dementia caregiver workshops as well as individual in home counseling. Tami is also a patient advocate, Certified Senior Advisor and is a popular and frequent keynote speaker on dementia and caregiver challenges and strategies. With ElderConsult, she provides One-to-One online education as well as speaking at monthly workshops.

Alexandra Morris

Alexandra Morris (Alex) is a gerontologist, with 30 years of experience working with people living with dementia and their families. She has worked in assisted living, an adult day program, director of a camp for brain injured adults and State Certified Ombudsman. She is a Geriatric Care Manager serving San Luis Obispo families regarding the challenges that come with aging family members and provided care consulting, community & professional education for the Alzheimer’s Association for 21 years.
